NH Transition Summit VIII November 18 in Concord

The New Hampshire Transition Community of Practice (CoP) announces the 8th Annual Transition Summit, the only statewide conference for training, collaboration, networking, and information focused on postsecondary outcomes for students:

Working Together: Increasing the Success of Postsecondary Transition Plans by Actively Engaging Students and Families in the Planning Process. 
November 18, 2014 | 8:30am–3:00pm | Grappone Conference Center, Concord, NH

This year’s summit will focus on the importance of active student and family participation in all aspects of transition planning.  Presentations will focus on:

  • Working Together
  • Creating a Plan
  • Achieving Outcomes & Next Steps

  • Limited scholarships available for transition-age youth through Granite State Federation of Families for Children’s Mental Health

In collaboration with this event, the NH Parent Information Center will be offering a full-day secondary transition training on November 22, 2014 for parents and families — more information to come!
