2022 Education Series

The NH Transition State COP has another robust lineup of hot topics and interest across New Hampshire in the 2022 Education Series.

The hourlong Zoom sessions are offered on the 1st Thursday of every month, 9:00 – 10:00 am, as part of the regular community of practice meeting. You are welcome to stay for the second hour, 10:00 – 11:00 am which is the regular meeting – lots of good resource sharing.

June 2 – Success in Healthcare Transition. Success Story of the collaboration within special medical programs for a youth and their family.
