The NH Transition State COP sponsored six presentations in the 2022 Education Series.

The hourlong Zoom sessions were offered once a month as part of the regular community of practice meeting.

June 2 – Success in Healthcare Transition. Success Story of the collaboration within special medical programs for a youth and their family.
