The annual Transition Summit is organized by the New Hampshire Transition Community of Practice. It is the only statewide conference for high-quality professional development on the practices that achieve the best outcomes for youth with disabilities as they transition from high school to post-secondary education, employment, and independent living.
This will be our 14th year of sharing transition-related best practices by and with New Hampshire educators, community partners, families, youth, and related service agencies.
This year the Transition Summit will be fully online. We will post more information as it becomes available.
